PLEASE NOTE: As of January 1, 1993 ALL ENTRIES must be on the official CKC Entry Form or a copy thereof. If you wish to submit entries on a computerized form it must be of the same size and format as the one issued by CKC and must bear the CKC Logo as per the entry forms below. No exceptions will be made.
The following files (in .jpg format) may be saved to your hard drive, printed for use & then clipped to size: 8 1/2" X 5 1/2".
